The first thing you need to do is to turn on Instant Payment
Notification - (IPN) After logging into your business account , click on
MyAccount -> Profile. Under Seller Preferences you should see Instant
Payment Notifications Preferences. Click on it, the click on the Edit
button. Check the checkbox to turn it on and paste this URL into the
input field:

This basically tells PayPal to notify the scheduling software that the
customer has paid and to process their appointment.
Make some sample appointments and ensure the correct funds are being
deposited and the appointments are being transferred. After you are
done testing you can issue refunds for all the transactions you made.
PayPal will issue a refund minus a 30 cent fee.
You should also set the return page to be some thank you/ confirmation
page on your website. Since the appointments take 10 seconds or to
process, you should send your users back to your website first. Under
Profile -> Website Payment Preferences, turn the auto-return on and you
can past the return url you like to use into the next field for return
url.
